Important instructions for compleating the electronic form:

• Be sure to have the latest update of Adobe Reader either by checking Help | Check for updates while Reader is open, or you may download the latest version from Adobe here.
• Save the file using the 'Save' icon on the upper left of Reader. I usually prefer to save such things on the Desktop, as there they are most easy to find.
• Open and complete the form.
• When finished filling out the form, click 'Save'.
• Send the completed form as an attachment by email to Bobbi Hopkins. If you prefer to mail the form, you only need print pages 4 and 5. The address is on the form.
• You may complete payment electronically through PayPal below. If you prefer to pay by check, the check should be mailed along with the printed registration form.
